Senior Survey & Scholarship Reporting - Final Transcript Requests
The Senior Survey and Scholarship Reporting window is April 1-May 2. The process must be completed by May 2, 2025.
Information collected from the Senior Survey and the Scholarship Reporting process is used for following reasons:
- Request (and consent to release) your final official high school transcript to the college/university you are attending in Fall 2025
- Data for the WHS School Profile sent to colleges and universities for college admissions
It is vital that seniors complete the Senior Survey and the Scholarship Reporting process on time due to printing deadlines.
How to Complete your Senior Survey
1️⃣ Log into Clever → my.nsd.org
2️⃣ Click Naviance under Learning Resources
3️⃣ In Naviance, go to Planner → Tasks
4️⃣ Find Complete the Senior Survey under “Tasks you need to work on” and click Take This Survey
📝 Important Survey Details:
📅 Deadline: May 2, 2025 – Be sure to click Save and Finish!
⭐ Required Questions: Complete Questions 1-4
🎯 Question 5 (College Decisions):

Update your list with ALL colleges you applied to.

Mark the college you’re attending in Fall 2025 and check “send final transcript” to ensure your transcript gets sent!
💰 Question 7 (Scholarship Reporting):

List only scholarships from the college you’re attending + any external scholarships (Rotary, Kiwanis, PTSA, etc.).

Do NOT include scholarships from schools you’re not attending, Pell Grants, State Need Grants, or FAFSA loans.

The total scholarship amount for the Class of 2025 will be announced (not individual amounts).

Submit proof of scholarships here: Scholarship Reporting Google Form. Proof is required to be acknowledge at Senior Awards Night on May 29!

Senior Survey Week
April 28 - May 2
If not yet completed, the week of April 28-May 2, is designated as a week for seniors to focus on completing the Senior Survey in Naviance.
- The survey takes no more than 5 minutes to accomplish.
- All seniors must complete the Senior Survey regardless of scholarship status.
Need to update your Senior Survey responses and/or scholarships after May 2?
Please email Ms. Tang (mtang@nsd.org) directly. Updates emailed after the May 2 deadline may not be included in the Senior Awards publication, however students may still be recognized during the Senior Awards Ceremony.
